The Blue Train name is inspired by the jazz legend John Coltrane and the name of his timeless album. He embodies the jazz tradition where deep technical mastery allows for creativity, responsiveness, and improvisation in the moment. Dr. Ed’s approach to psychotherapy works in a similar way. Effective treatment is grounded in rigorous evidence-based methods—the Fixed Values. But no single approach works for every individual. Real progress requires Flexible Treatment: the ability to adapt—thoughtfully and skillfully—to each person’s unique life circumstances. At Blue Train Clinic, this means combining strong clinical structure with the flexibility to adjust the approach so it truly fits you.
This is not a high-volume practice. Dr. Ed works with a limited number of clients at a time to provide focused, individualized care — especially for those dealing with complex or hard-to-treat conditions. The goal is not ongoing therapy without direction, but meaningful progress and forward movement.
The first step of the journey is an evaluation focused on understanding your situation and building a treatment approach tailored to you.
Therapy sessions are 60 minutes (not the “45-minute hour”). The longer session is a more efficient way to make progress and give your concerns the full attention they deserve.
Therapy requires an investment of time, money, and emotional effort. The aim is for you to get high value for the money you are paying.

It is not a general practice. Blue Train Clinic is a boutique practice where Dr. Ed specializes in hard-to-treat cases. People who seek treatment with him have usually already tried therapy and have not gotten the results they wanted or needed. He brings deep clinical experience in evidence-based treatments, with Clinical Psychology Board Certification and 25+ years of clinical leadership at Harvard Medical School. Over the course of his career, Dr. Ed has provided direct service and led programs for thousands of patients with Depression, Anxiety, Bipolar Disorder, and other complex mental health conditions.
No. Blue Train Clinic is self-pay only. Dr. Ed is not on any insurance panels. Dr. Ed customizes the therapy for each client and is not constrained by insurance-based restrictions and administrative barriers.
Fees reflect the Premium Services Dr. Ed provides with his experience and credentials. Fees include 60 minute sessions, report writing, customized education materials, customized audio recordings for skills practice, and coordination of care as needed. Fees for additional time/services outside of therapy sessions are determined on a case-by-case basis.

Dr. Ed works with adults dealing with Treatment Resistant Depression, Severe and Debilitating Anxiety, Bipolar I and Bipolar II Disorder, and young adults experiencing first-break bipolar or psychotic disorders. He works with parents and family members who are supporting a loved one through serious mental health challenges. And, he offers Family Therapy to guide and support families in crisis or with ongoing challenges.
Every client starts with a thorough evaluation. From there Dr. Ed builds a written treatment plan tailored to your specific life circumstances, history, and goals. He draws from CBT, ACT, DBT, Mindfulness-Based Stress Reduction, Behavioral Activation, Psychodynamic Therapy, and Family Systems Therapy depending on what fits you best. Progress is reviewed on a regular basis which combines Dr. Ed’s assessment and the client’s feedback. Adjustments are made and the treatment plan is updated until the next review.
Based on the evaluation, Dr. Ed reviews the options with you. Standard therapy is once per week. A “Frontloading” approach is two sessions per week to gain traction quickly. Given the circumstances of many clients frustrated with past treatments, Frontloading is desirable because it tends to produce results more quickly. Once you have positive momentum you decide whether it makes sense to taper down the frequency of sessions.
Dr. Ed is based in San Diego, California and offers telehealth sessions for clients in California, Massachusetts, and Florida. He also sees clients in-person in his office which is centrally located in San Diego.
This is very common. Dr. Ed often has first contact with parents seeking treatment for their adult child, or other family members seeking treatment for parents, siblings, or extended family. Many young adult clients dealing with bipolar disorder or early onset psychosis are resistant to treatment and parents are often the ones reaching out first. Dr. Ed provides education, guidance, and support for parents and other family members who are navigating this complex family situation.
